Customs Intake

Please fill this Customs Intake form to inform me about any custom you'd like to inquire about having created! Custom offerings include altar item, wearable amulet/jewelry, canvas painting, or any idea you may have! Once submitted, look out for a message from me so that we can move forward in the process. If you're not ready to start the process of creation yet, you can still fill this form, for free of course, and we can circle around to it in divine due time. There will be an intake price of $17.77 for us to get on the phone and start curating the vision after this form is submitted, that way I know you're really ready to get to co-creating a masterpiece! This price will not be additional to the total price, so the $17.77 paid will roll into the total!
